I have a machine with a 9.1B Software version, it keeps failing at DFS 12200 :/ PDA reading is around 350 mmHg NSVP is high

Disconnect the MSBK1/2 sensors on the side on the balance chamber and check for any splits in the membranes ferrite rods, could be a balance chamber issue. When its doing its test check the sensor values are changing in sequence together, if they are all over the place you will be required to replace the membranes for new.
Also check the pump speed of FPE, possibly could have a worn pump, you can test the pump the same way you'd test RVFPE, setting the speed of the pump to get a pressure reading from RVFPE, if its a lower value then it could be a sign of a worn pump
Your PDA value looks spot on and aslong as the level sensor in the tank is switching between low-med-high in test 1.09 then ii would be safe to assume PDA and NSVP are not your issue here

Go to TSM low level test 1.13. Open all valves VEBK1/2 and VABK1/2, Close VDEBK1 / 2 and VDABK 1 / 2. With a syringe in front of FBK1 build up a pressure of 1.3 bar. You might have to repeat it a few times because the volume of each chamber is 100 ml. If the pressure remains stable on the manometer, it means that the membranes are not leaking.
