I came in this morning to find a dialyzer (unused) inside a plastic lab specimen bag sitting on my desk. No note or anything. Anywho....what I found is that there is a piece of what looks like green paper inside one of the headers. This was a Revaclear 300 and Baxter has been notified.

Chuck, thanks for the heads up on this. We use Revaclear 300's and 400's as well.

Our gloves are blue and, its definitely paper.

unless you are familiar with the manufacturing process you will probably never know what it is or how it got there have you notified the maker and possibly sent it back to them for analysis

A few years back, I got a pack of tubes from Ascend. When processing my samples, particularly affixing the labels, I noticed something didn't look right. There was a brown piece of what looked like paper towel floating in a tube. Sent them pics with the lot # and all that. Never heard back from them.
NOW I am paranoid and invert every sample tube as I am labeling it! I was just thinking the other day how I don't think I have *enough* to worry about...rme
