This isn't really a technical question but I'm sure I can get some answers here. Attached is our current Machine disinfection log. It has multiple machines on each sheet. It is really busy and hard for staff to read. I have errors on this thing weekly. Would anyone be willing to share the format that you use for this. I had thought about having a monthly log for each machine but with having over 40 machines, that is a lot of paper.

I have over 40 in my clinic and we went with the 1 page/1 machine method. Put it in a 1/2" binder and it has worked well for us over the years.

One sheet per machine with the months back to back and hanging in a protective sleeve. The information is the same as yours except we have space to document such things as machine down, so that any surveyor would understand why a particular machine wasn't disinfected. It does create more papers, but I think there is less chance for errors or fakery.

We kept a visual written copy of spreadsheet on the folder stuck to each machine.
The spreadsheet has five columns No. Date Hot Disinfection Rinse Signature
Also every time before use, we also check machine cleaning history that it has been disinfected between treatment.

Earlier email was example on Nipro Surdial X machine we use on some sites that only required hot disinfection not bleach which is quite similar to some sites using Bbraun machine.
Rest of our sites which use Fresenius machine will have extra column which is 'Bleach/Disinfection ' on our spreadsheet. I believe once the spreadsheet been populated , the unit manager will scan it to be saved as soft copy archive.

where on this sheet do you mark for the water loop disinfectant(chemical) both positive and negative?

where on this sheet do you mark for the water loop disinfectant both positive and negative? I have a separate sheet for that. It goes along with my RO/Loop disinfection sheet. I sign off when I check each machine and then staff does a second check in the morning.
