We've recently had 5 incidences where the pressure line from the arterial POD was obstructed/glued shut (Lot #00VL862418). To compound this problem, because of the programing, the system passes the blood side pressure test and allows the treatment to be started. Shortly after the treatment is started the machine detects that there is no pulsation in the arterial pressure and alarms (as it should). The alarm can be reset to restart the blood pump which naturally alarms again. Depending on how many times the clinician resets the alarm trying to troubleshoot the the issue, the blood loss could be substantial.
You can easily simulate this by setting up a machine to do a mock treatment and putting a clamp on the arterial pressure line. Note that if there is a problem with the venous pressure line, the machine will not pass the blood side pressure test.
The machine is operating as designed but IMHO, that design is flawed and allows patient injury. A PIR is filed with Braun and I will wait to see how it is handled before filing MedWatch reports.

I have also been having problems with the streamline at my clinic. I contacted Braun and sent them the lines back, but now its happening again with another lot number. How do you contact MedWatch to make a report?

Hi all, I am having some issues, where lines are badly kinked specially blood pump segment. I have been reporting to BBraun. Does anyone having any issues like minor blood leak from bloodpump segment during treatment is running ?

Does anyone having any issues like minor blood leak from bloodpump segment during treatment is running ? Earlier this year, we had 2 incidents. One was a very very very tiny leak in the pump segment and the other was from the glue joint where the heparin Tee attaches to the main tubing. You use the Streamlines for Fresenius machines don't you?

The problem I'm having is micro bubbles are in the lines and doesn't matter have tight the lines are there is still micro bubbles. Also at the end of treatment when you are rinsing the patient back air gets in the air and starts clotting the blood where we cant return, and the arterial lines keeps breaking right under the bubble.
