Has anyone heard anything about Fresenius no longer sending dialysate port caps with the dialyzers?

Urgent Memo
Purpose
The purpose of this memo is to inform Fresenius Kidney Care (FKC)facilities and programs that Fresenius Medical Care Renal Therapies Group, LLC (FMCRTG, LLC) has made a change to the port caps supplied with the Optiflux® E-Beam dialyzers.
Background
Fresenius Medical Care Renal Therapies Group, LLC (FMCRTG, LLC) has made a change to the port caps supplied with the Optiflux® E-Beam dialyzers. Customer Letter
The dialyzer blood port cap is the sterile barrier on the blood ports of the Optiflux® E-Beam dialyzers. FMCRTG, LLC has adapted this cap for use on both the blood and dialysate ports of the dialyzers.
The user can move the cap from the blood port to the dialysate port just prior to attaching the bloodlines. The caps can seal the dialysate ports during priming and can be reattached post-treatment to prevent leaks during disposal. This dual-purpose cap has allowed us to eliminate the two adaptor caps placed within the dialyzer overwrap.
The cap should be pushed not twisted onto the dialysate port of the dialyzer. There will be a tactile click the first time the cap is placed on the dialysate port.

We are experiencing some issue with these caps. Not so much the caps as the ease of slipping through the fingers and straight onto the dirty floors. So much so that staff has resorted to hoarding them. We put a quick stop to that, but we have to come up with a better solution. I have been looking into reuse caps however, I don't think they come in individually wrapped packs.

So, from the horses mouth ( custy service) , there currently isnt a way to order caps individually as of yet, but from what I gathered, there most likely will be in the future.

So they stop including the extra caps but will offer them for sale because they are still needed.................

Happy Friday all! well, when i was talking to the custy service rep she said that *alot* of nurses complained about what to do once they drop the cap, so they'll prob change it in the future. if its one thing iv learned, big F will do anything to keep nurses happy. they get like 20 kinds of scrubs for their free uniforms. Biomeds? we get to pick from 3 kinds of shirts. just saying. nobodys thrown me a free lunch as a morale booster.

Can we keep the dialyser in horizontal position during dialysis ? Does it affect the clearance or any other parameter ? please explain. Thanks

@ramanathan you cannot. during dialysis the only designated position is with the arterial going up verticaly. this is to allow any air that could get trapped to be pulled off to midagate clotting, and keep the counterflow stable. this is listed in the current 2008T Machine Operators Manual, P/N 490122 rev Z, Page 135 in the "Starting dialysis" section.

Rev AA is the current user manual and it states to place arterial end up if desired. This is when you refer to your companies policy and procedure and follow that

This is not true. If the arterial is up, any air present would be trapped in the header causing clotting. In order to be drawn off, the venous end needs to be up so that any air present would pass through the dialyzer ending up in the venous drip chamber from where it can be drawn off. Also if Streamlines are being used, their IFU is to have the venous end up for this very reason. And yes, Streamlines are used on Fresenius machines also.
