Hi all, recently I witnessed a biomed at another facility fudge the numbers on a bc volume calibration in order to get a lower condo. It was set 2.0 lower than what was written on the bc. Can anyone provide any info on this topic? It was a bibag machine and this was for PM purposes according to him. I personally do not like to type in numbers that do not match what I am getting.

I too, refuse to do that. It's not necessary. By fudging the volume you are changing the prescription. I do not recommend you do this.... over the course of one treatment how many times does the balance chamber switch??? By changing the volume by 2.0 minus the actual volume over the course of one treatment that equals a significant difference in the volume that what we are actually givng the patient. In most cases if you recalibrate the acid, bicarb, condo cells and the condo confirmation cell(using the Yellow cable and dongle)the machine will be roughly .1 higher than TCD(Theoritical conductivity of Dialysate). Sometimes it will go to .2 above. There is no need to change the balance chamber volume to MAKE THE MACHINE run in line with TCD... Just my two sense.. In most cases where i'm covering a clinic and I notice that this is happening I change them all back to what is on the balance chanber and recal the above items to get them back to normal. I also usually find that the wall box pressure of the acid and bicarb is too high causing the machine to run high. Simple fix if you have regulators. Not such an easy fix if they don't have them... Either way it is a challenge!!!

I've seen people do that, seen people "trick" the acid and bicarb pumps to adjust the conductivity to be in line with TCD, personally, I don't do that. In fact, the few times I've seen it, I applied a little peer pressure to do calibrations the correct way.

It's too bad the acid and bicarb pumps can't speed up or slow down automatically to obtain the perfect conductivity.

Theoretical- assumptions are made to give you a conductivity value, in real life, there are no assumptions and each instance is unique. One of the instructors that now instructs at the Coppell training facility wrote that on a dry erase board when asked about TCD. I wrote it down and use it to explain to those that ask why this machine doesn't match TCD perfectly.
