Check the drain line when it is in "Circulation" and see if water is coming out. You might have a bad valve. Also sometimes you get a leaky incoming water solenoid but that would slowly overflow the tank and you would see water coming out of side of bottom of machine.

did you check RVDA for the correct setting, it should be around 300 mmHg. Next thing to go for would be the VZ valve check it in TSM if it is opening and closing properly.

What machine do you have running what software V8 or V9.12 or V>9.12? Different versions operate under different rules!
How many hours are on the machine? This helps with identifying what items could possibly be causing your problem.
What troubleshooting have you done so far. Have you tested PE for proper calibration? Do you have an RVE or VEB?
Did you check to see if the short silicone tubing between the straw and the coupler nipple is the proper size and length? If someone put any other size on as a quick fix, they may have created the problem. Oversized tubing will swell with the hot water restricting flow thru the wand holders which causes a back pressure felt by RDA causing it to open an drain the disinfect solution. Eventually, fresh water enters and dilutes the solution to the alarm point.
Please post more info.
