Have you checked the inlet pressure with the machine off or flow off and the water on? The pressure shouldn't spike. If the pressure spikes, the inlet pressure regulator needs to be replaced or rebuilt. My other thought would be a leak in the heat exchanger.

Have you check the vent/check valve ( half greyish and cream colored filter - valve 92 i think ) is inserted the wrong way ?
I think grey side is to connect to the venting hose , cream side is connected to the beak tank.
Maybe change the check valve and check loading and degassing/aeration pressure too.

Very likely heat exchanger is bad.

Sorry for the delay in response. Valve 92 is good. Heat Exchanger is also good. Rebuilt the inlet water regulator. Leaked 4 hours after machine completed bleach disinfection.

How are you determining that the parts you mentioned are good? If you look at a flow diagram of the machine, there aren't that many places that are under pressure when the machine is off.

Used to have a staff member that would "fix" this with a Kelly clamp ...(NOT recommending this, by-the-way)

Just seconding the other comment: how do you know the Heat Exchanger is good? Leaking from the vent during heat, the very first place I look is the heat exchanger. It’ll operate “leak-free” in every other mode and just leak as heat continues.

id say its your heat exchanger. the only true way to test your heat exchanger is to turn on HE test in the service menu. BUT one of the symptoms of a bad heat exchanger is fluid coming out of the vent tube during heat disinfect. yes it could be other things also but id start at the HE. I have had to change multiple of my machines heat exchangers in the past year and most of them were less than a year old. plus its not a super expensive part and its easy to install.

Update: After replacing Heat Exchanger, Inlet regulator, and recalibrating all pumps, still had leak from behind. Replaced valve 41. No issues with leaks since. Thank you all for suggestions.

8 months to fix this? I hope you did a culture and endotoxin on this machine before putting it back in service?

Awesome! Thank you! Had a leak from the vent during heat. Valve #41 fixed it!
