For about the last month or so, we have had 4 Lot #'s of bloodlines where SOME (not all) of the venous lines are blueish in color and causing blood detected in prep mode alarms. Our latest instance this weekend, the facility used 8 out of 10 cases of a lot # without any issues then started having problems. I tried to attach a picture but it's not uploading but the discoloration is clearly visible. The Lot #'s have all started with A2200***. The most recent being A2200777. Has anyone else been having issues? Yes, this has been being reported to Braun.

I was contacted by someone from my corporate office regarding issues with the venous line looking "cloudy". I'll shoot you a pic.

None of the blue color problems, but we did report multiple issues with the venous tubing being stuck to the side of the leurlock connector instead of inside the connector. This was reported also but the problem was across multiple boxes of the same lot. A2200722.

Haven't seen it but I only use bloodlines when I have to string a machine, and if I haven't used the lines in one week I re-use, after a week I get new ones. I'll have to check our stock on monday though, thanks!

Hi Chuck, I am having issues where blood pump segment is seriously kinked due to poor packing and we had few incident where pump segment had a minor cut. I think Bbraun needs to review and reevaluate their packing. ( bigger box or lesser count ).

New issue with tubing sets. Heparin tubing occluded out of the package. No patency and unable to get heparin to the blood. Had 4 last week and 2 this week. Lot number is 00VL862270
Nurses thought i had 4 heparin pumps go out in 1 day.......heparin alarms all over the place......

We have now had a new issue also much like yours but with the arterial pressure tubing from the POD. What makes it twice as bad is that the machines pass blood side pressure test. It's not detected till the treatment starts, after aprox. 15 seconds or so the machine alarms because it doesn't detect the fluctuations in the arterial line...unfortunately, the patient has now lost blood. Our affected Lot is #00VL862418. A CIR has been filed for both the lines and the machine.

I submitted as well.... Just an FYI.... I tried to send a CIR in December 22 and this is what i was told.

The Product Quality Excellence group is a useless joke. I file mine through Tech Support or in the case of the blood lines through my sales rep.

Just to be fair, the Product Quality Excellence Group (PQEG) is B Braun's Interface with Germany's different divisions (surgical, infusion, HD, CRRT, Plasma Apheresis, Streamline bloodlines, CAPS Pharmacy, Asculap, and third party vendors. It is their job to take in a "completed" CIR (customer Incident Report), parse it into the different groups of data to create report(s) and send them to the quality control departments of the product in question. It is not their job to take in and record your problem report.
So who can process your complaint? Your local or a known B Braun Field Service Engineer (FSE) (Dialog+); a member of the clinical team (bloodlines etc.); B Braun sales representatives; "Lab technical support". There is a specific form to complete to take in details, record if samples are available (a quality department may want it for inspection), patient injury (FDA gets involved), etc.
If you have a problem, reach out to your local FSE or call the Lab. The only time you should communicate with PQEG is when they request you to ship a CIR item off for investigation. And they will supply you with all the documentation to ship it out for free.
